James has been diagnosed with Von Willibrans Disease, which is a blood disorder. The blood doesn't have enough blood cells which means that he can bruise easily. VWD is usually hereditive and Michael and Rebecca have found out that Michael's mum and sister had it. This means that Sarah might have it and if Sarah gets it then it's almost certain that their other children, if they have any more, will get it. It is nothing to be worried about but James must not play contact sports. If somebody accidently hits him in a game, he can bleed to death.
