Those aren't actually their names, that's just how they're called in history, or sometimes how they were called contemporaneously.Many referred to as "the elder" was just to differentiate them from their junior (sometimes called "the younger"), and those referred to as "the Great" were mostly because they brought about many good changes during their reign. Sometimes it's just a nickname from a description, such as Pepin the Short or Richard the Lionheart.
