Luke can and very often does stand on its own. I don't especially like it but it's not bad. Luke Anderson seems better than Luke Addison; Addison is one of those colorless, androgynous surnamey names for girls that are very popular right now. Anderson has a bit more backbone and is more emphatically masculine than Addison.
I really do not like the name Lucy. It sounds goofy and cheap and wannabe-posh all at the same time, and Lucy Kate is so damn precious. Lucia I like, and Lucia Kate is okay. I pronounce Lucia like Loo-SEE-a; there's a couple different ways it's pronounced and that alone is a deal-breaker for me, though I do like the name.
I think it's fine to use the nn Kate as a middle name.
