As a Scottish person, I wouldn't recommend this for a first name. For me it'll always be a surname as there was a [first name] Balfour I knew growing up - I hated him, but that's not the point.
I don't know, for me I'll always see it as a last name, but maybe in other countries.
As far as I know, R. L. Stevenson is sometimes referred to as Robert Louis Balfour Stevenson because his mother's maiden name is Balfour.
This is the surname of David Balfour, the hero in Robert Louis Stevenson's classic novel "Kidnapped".
