I thought the multiple choice CAF that Eirena did was great, so here's another. Hope you enjoy! (It's includes friends of family too!)
LN: your favourite colour is:
Blue - use surname of favourite teacher
Yellow - use surname of best friend
Red - make up a surname using 3 consecutive letters from your own
Green - make up a surname beginning with the first letter of your first name
DH: you are from:
Europe - FN same nationality as your FN, MN choose
Asia - use initials BJ, B must be more syllables than J
Australia - initials of FN & MN must be the same
Americas - use your own initials, match nationality of surname
Africa - FN 1 syllable MN 2/3
DW: you are:
15 or younger - FN colour related, MN a character from TV
