This is almost assuredly a name created from ancient Greek, though whether or not it was ever used as a person's name in Greece, I don't know. The eu- part is a common prefix in Greek names which means "good". Perhaps Pavlos will show up to explain the "phorm" part to us. :)Of course, it could just be a blend between Euphemia and the plant name "euphorbia" created deliberately or accidentally by someone who got a bit confused.
