name
I would like to know where this name originated from?

This is almost assuredly a name created from ancient Greek, though whether or not it was ever used as a person's name in Greece, I don't know. The eu- part is a common prefix in Greek names which means "good". Perhaps Pavlos will show up to explain the "phorm" part to us. :)
Of course, it could just be a blend between Euphemia and the plant name "euphorbia" created deliberately or accidentally by someone who got a bit confused.

Okay, well, you didn't really specificy so I am guessing you are looking for the origination of the name Euphormia. I am also guessing it is a female name.
I would guess that it comes from the male name Euphranor. If that is not it, it could be a mix of common words like Euphoria and something else.
Next time, give any information you know of, like your gender, perhaps meaning, and the specific name you are looking to get information on. It would help us helpers out...lol!
