...Her given name is Destiny Cyrus, but her father (Billy Ray of "Achy Breaky Heart" 'fame') called her Smiley as a pet name, which degenerated into Miley.
If there are other, etymological explanations for the name (in this particular spelling, rather than the sound-alike (I think?) Maile), I'm not sure, but that's how the most famous bearer came by it.
