It's a boys' name to me because I automatically think of River Phoenix, but I can see it on a girl, too (but it's just nms).

It seems better suited for a boy, but it works for either gender. I would use a very masculine or very feminine middle name to avoid confusion.

To me it is truly androgenous. It would be equally appropriate for a boy or a girl. I might prefer it on a girl, just because I am big on strong names for boys and River seems a little hippy-ish/soft.
