I prefer it as a nickname for Madeline or Maddalena, unless the bearer is a pioneer woman. Maddie and Mattie both have a folksy, olden days appeal that I appreciate in the right setting. I agree with Rox and Wordsmith that it's one of those neo-frontier names that have now become synonymous with spoiled pre-teens, all the folksiness is sadly gone.
