Sylvia! But I like both. I know this is not a popular opinion here, but I think Patty and Patsy are really cute.
Also, your grandmothers had much nicer names than mine: Mildred and Blossom. Oy.
Middle names I'd guess something like Mary or Louise or Anna.

Patricia Marcelaine is fantastic! I think there's a name nerd in your family tree.
My pick would have to go to Sylvia. Not because it happens to be the name of one of my nieces, but because I've always found it elegant and classy. A time-honored name that's suited to all ages and a variety of backgrounds. Patricia on the other hand I don't mind, but it's definitely more on the dated side.
None of them are all that attractive but I’d say it depends how I’m feeling.

It's graceful in a dreamy, comforting way. Patricia is so drab. It's also one of my grandmother's names, but she can't stand it, so she goes by her middle name, Wendy.

Much prefer Sylvia, it feels more timeless.

Definitely Sylvia! I love the meaning and overall feel of the name. Elegant, strong, beautiful, and a little ethereal. Patricia is a bit of a meme in something I watch, so I can't quite take it seriously.
For middles I'd guess something straddling classic, dated, and vintage. Maybe Sylvia Anne and Patricia Jean.

I'd guess one is Ann/Anne and the other maybe MaeMay? Also-rans might be Jean or Jane and maybe Marie or Louise?
I like them both. I think Patricia is a little bit brighter and more cheerful, and if you want nns, there's a good few. Sylvia is pretty, but the only nn I see is Sylvie, which feels weak to me.
