I love it. I have met a Hermione who was 15. I don't think you'd have to correct people very often because Harry Potter has made this name more recognisable.
The only problem I have is that it is rather long and there aren't realy any nicknames which are as lovely as Hermione. Herm or Hermy are awful. Mia might work, but it doesn't do justice to Hermione! Ione is nice but I'm not sure if it works because Ione isn't pronounced the same as the end of Hermione.
I would probably use Hermione as a middle name. I like the combo Freya Hermione, Eve Hermione and Luna Hermione (although the last one is probably a no-no!).
