I would immediately think the parent(s) was/were trying to be different, thinking that by changing the spelling of the name, it would change the name itself (those troublesome creative spellings!).
Bethanie looks awkward to me, while Bethany looks much simpler. With the spelling Bethanie, there would probably be spelling problems, as people would most likely assume the spelling is Bethany.
I think it's a lovely name, with the spelling Bethany only.
P.S. For some reason, I see parents who really want to be cutes-y with their twin girls, by naming their daughters Stephanie and Bethanie. Haha.
