I like Lazaro and Lazar better. Lazarus seems too stuffy and tied to the Bible story.

i named my computer Lazarus (because of how many times it's died and been brought back to life), but i dont think i'd use the name on an actual person

I like it, but I think it comes with too much baggage because of the "Jesus raising from the dead" thing. Because of that, it makes me think of mustiness and rotting flesh. Even though Lazarus was supposedly fresh as a daisy after he came back from the dead. But I still like it! Yeah, middle name slot would be good.
