I automatically think of it as a girl's name. I've never heard of it being used on a boy before.

I read a book ("Fried Green Tomatoes at the Whistle Stop Cafe") in which the narrator's husband was named Cleo. We also had a male resident at the home named Cleo; I think it was used for the boys way back when, but has shifted to the girls as the years have gone by.
