I can't think of a time in recent centuries when it was common enough to sound old-fashioned. It doesn't grab me enough to use it myself, but I would find it a refreshing change from Sophia, Emma, Madison, etc. if I met a little Keturah. It's not overused, but is a real name (and a pleasant enough one @ that), so I would consider it usable if you wish to have it on your list.
