This is my Poly-family. Mother's (I added 1 mother) have kept their last names while taking their husband's too, and the kids born has their birthmother's last names as well as their father's.

I used the count-along method, using a 10-sided dice for names (then tweaked them to my liking - switching names etc) and a 6-sided for last names from a multiple dice app on my phone, naming kids first, parents next and then last names. :)
