If you look under the name Guinevere, it says "from the Old Welsh whatever Gwenhwyfar, which is composed of gwen and hwyfar etc. etc." A lot of times Mike C. seems to simply list the original form of the name in with the most common close variant if the original is very very ancient and/or obsolete.
I happen to think that Gwenhwyfar, being the most authentic and most Welsh form of the name, is absolutely lovely....but since other entries similar to this situation do not have separate entries, I think I would recommend being content with having it listed under Guinevere as the original source of the name.
