If I wanted a Mary-Kate, which I don't (don't enjoy hyphenated names in general), I'd go for Mary Katharine and just let Mary-Kate happen or not. It's got the same faux-family name sound as, say, Pollyanna. And it's probably that vintage too: Pollyanna or even Anne of Green Gables might well have a Mary-Kate friend.
