It definitely seems southern U.S. I would definitely confuse it with Willadeen/Willadean. I think if anything it should be broken up with Willow as a first name and Dean as a middle name, but I would much prefer Willow, Willa, or Wilhelmina.
I encountered this name a while ago. I think it's okay, but it doesn't really flow well, as Willow is a very modern name while Dean is a bit old.
It's very whimsical Southern. I would instinctively pronounce it like "Willadeen", which gives it a nice warm twang. I like it. Much more than Willoughby, in terms of Willow elaborations.
