How does she know they're identical before they're born?
The names are both nice. I especially like Savannah. Sophia is not a favorite, but it is pleasant. It fits with Savannah but its style is quite different so they don't strike me as matchy-matchy names.
Grace and Hope as twin middle names are very cliched.

Sometimes identical twins share an amniotic sac and/or placenta, which doesn't happen with fraternal twins. If the doctor identified that they shared, then it is most likely they're identical.
I only ask...
Because I've never heard anybody say they were going to have identical twins, just that they were having twins or twin boys.
