I think Bianca is a beautiful name and I have a close friend called Bianca, whose parents are from Hong Kong. Bianca looks a lot nicer than Bianka, I think, and you definitely would have to spell Bianka out every time. I really don't think Bianca is too weird for someone who is not Italian.
Bianca seems a bit out of place with sisters called Margaret, Catherine, etc., but just a bit.
Bianca goes all right with Vivienne, but I don't like the name Vivienne much. Middle names I think work nicely are Rose, Alana, Olivia, Marcelle and Valentina. Cross cultural middle names, especially other European ones would work fine.
