1: Juliet, pronounced JU-lee-it. Very lovely; I'd use it as a mn.
2: Julia. Wonderful tradition, but I wouldn't use it.
3: Juliana. Very stodgy Dutch royalty, and always cut down to the boring Julie. No.
4: Juliette. Fine if you're French, silly if you're not.
