1. I think it's alright. A bit old-fashioned, but I like those kind of names. I have a friend my age (early 20's) named Sally, so she kind of puts a new twist on it for me though.
2. Anything else old-fashioned... Nancy, Sue, Betty, Fred, Eddie (got those from Nymbler) :)
3. I think it would be best as a nickname. Probably for Sarah. I don't know what else it could be a nickname for. Any other Sal- or Sar- name, I guess.
4. Sally Louise is what comes to mind (I noticed BBH posted it). My husband's grandmother's name is Rachel Louise, but she goes by Sally.
5. Yes, I do. As I mentioned, I do know someone younger than 30 named Sally though.
