To answer points 1 and 2:
I've always preferred Katy. It seems more grown up and less "cute" than Katie.
I quite like it but don't love it. I don't have a problem with it being a nickname but prefer Kate. I'm getting bored of little Katies though. It's so popular over here and has been for over 20 years.
