I've read a book with a girl named Niamh in it before. There was no pronunciation guide, but the pron. was emphasized in dialogue early in the book. That's my favorite way to introduce a difficult name - have a character fumble over it and be corrected along with the reader.
I can pronounce Niamh, Caoimhe, and Siobhan just by looking at them, but I did go through an Irish phase a few years ago and learn some of their pronunciation rules. :)
