I much prefer it as a nickname. It seems a bit insubstantial as a first name. I'd much rather see Katherine/Catherine/Katharine nn Kay or Kathleen nn Kay. I also slightly prefer the Kaye spelling. Thoughts on your combos:

Kay Bellatrix - Bellatrix is far too strongly associated with HP right now. It's one of those names, like Hermione, Sirius, etc, that needs some time. Would Kay Beatrix work for you? Kay Beatrice? Kay Bellona? Kay Belphoebe?
Kay Lavinia - Not bad, though Lavinia's not a name I love about.
Kay Penelope - Very cute. I like it.
