I'm from Holland too, I didn't even know there was a cookie named milano. When I hear Milano I think of the city, and I love italian city names (among my favs: Ravenna and Verona :-)

I don't think we have a cookie of that name either (UK), but I think of Alyssa Milano and of the city Milan. I dislike most names ending in "o". Anyhow, it's unusual which is a good thing :-)

I think of the cookie and of the surname (as in actress Alyssa Milano, like Zuzu said). It isn't at all masculine toned, and I don't think it feels like a name.
With a second name, do you mean a second-first name or a middle name? Either way, I suggest Milano James Henry (two-first names, Milano and James) and Milano Patrick (Patrick being the middle name).
