I know a Jeremiah who everyone calls Jerry

Jeremy *is* a name on its own, regardless of whether it can also be used as a nickname for Jeremiah.
Personally I wouldn't bother using Jeremy as a nickname for Jeremiah, since it's only one syllable shorter. I'd use Jem for both.
