Ottavio (ot-TAH-vyo)
These are the names of two of my great-great-grandparents, The only ones that I know (I have no idea of any other distant ancestor).

Ottavio is really neat. It's one of those very Italian names that I love, but would feel silly using myself since neither DH nor I are Italian.
I like Carolina (your pronunciation) but I always read it like the state. It would take some getting used to.

I love Carolina
I like Octavio / Octavian, and the Ott* in Ottavio makes it seem a bit subtler/softer. Also, it reminds me of Otto.
I've heard Carolina pronounced that way, but I'm more used to the English pronunciation, which I strongly associate with the American states. I slightly prefer it (with the Italian pronunciation) to Carol and Caroline, neither of which I like much, but I don't dislike them either.
