As a full name, it's pretty stereotypical redneck. You'd expect a Mack to be known as Mac Ray or Mack Lee or Buddy Mac or something.
I think it's best used, if at all, as a nn for either a first name like Malcolm or MMark or Michael, something like that, or for a person with a Mc/Mac surname. I did know somebody in high school whose last name was McCarthur and who was called Mack.
