I don't know about frequency, but a lot of people I'd imagine stick their middle initial or first initial (if they go by their middle name) in their professional/public name just because it sounds more professional and academic and complete. That's why/when I'd do that, anyway. Not really any other reason.

I use my middle name simply because I prefer it, but since most of my documents, accounts, credit card, etc. will only use my first name, I sometimes use K. Fiammetta Lastname to avoid confusion.
I have seen single letters on birth lists, though (lists of all births for one specific year in one specific country, for example.)
