I went to college with someone from Mexico named Lilia and her nickname was Lily. I think Lilia is really pretty and I prefer it to Lily. I pronounce it as LIL-ee-ah and LEE-lee-ah, depending on the on the person/language. I don't think it would become common.
I prefer Lily, really. Lilia looks unnecessarily elaborated to me.
I definitely think there is the possibility that a Lilia would be confused with Lydia and Livia especially. With the way all the Lily names are charting (Lily, Lilli, Lillie, Liliana, Lillian etc etc etc, even Leila, Lila, Lyla and so on) I can see an interest in Lilia forming.
Off the bat, I'd pronounce it like Lily + uh.

She has a sister named Holly.

And, no, they won't assume that she's anything else...my classmate is never assumed to be anything else...the only named I know of that people assume is something else is Edmund...I know a kid by that name, and he says some people call him Edward.
