5. Based on number 6, I'm assuming you're asking for girl combos, so here goes:
Blair Michala (mi-KAH-la)
Blair Rosetta
Blair Catriona
Blair Arietta
Blair Alexandra
Blair Emmeline
Blair Jemima

Blair Matilda
6. I think it works better as a FN, for a girl anyway. I find it incredibly odd on a girl, and if it was hidden away in the MN spot it would seem even weirder, especially if the first name was quite feminine. Not a fan of Caitlin Blair.
