I am sure I have seen the Smitts, John III form (and not III Smitts, John) in the usual context like telephone lists. This also makes sense in those contexts, because this inversion in English is so that names come out alphabetized by the last name. The next part is usually the first name, preceded by honorifics like Mr, Mrs, Dr, etc. and then follow all the qualifiers like middle initials or names, sr, jr, II, III etc.That is just what I am familiar with ...
